If you are moving away from the area or planning to switch to another competitive club, you need to submit the following form by the 5th of the month, to avoid owing team fees for that month.   If you are switching to another competitive soccer club, you must select the option on this form for NCFC to release you, so that we can notify Norcal and drop you from our roster - before you are eligible to play for another club.
